The HTV-3 Japanese cargo spacecraft was successfully captured with the International Space Station?s Canadarm 2 robotic arm, and then installed to a docking port. JAXA astronaut Aki Hoshide berthed the HTV supply ship, called Kounotori3, or ?white stork,? at 14:19 GMT (10:19 EDT) on July 27, 2012 to the Earth-facing side of the Harmony node on the ISS.

Earlier, working from the robotic workstation inside the station?s cupola, NASA Flight Engineer Joe Acaba, with the assistance of Hoshide, captured the 16.5-ton cargo ship with the station?s Canadian Space Agency-provided robotic arm, and as the spacecraft flew within about 12 meters (40 feet) of the ISS.
The unmanned cargo ship is 10 meters (33 feet) long and 4 meters (13 feet) in diameter and is capable of delivering both internal and external supplies and hardware to the station.

The name Kounotori was chosen because it the ship?s arrival represents an important delivery. The space station crew later opened the hatches and began the process of removing about 3,175 kg (7,000 pounds) of supplies from inside the Kounotori3?s Pressurized Logistics Carrier. That cargo includes food and clothing for the astronauts,?an aquatic habitat experiment, a remote-controlled Earth-observation camera for environmental studies, a catalytic reactor for the station?s water regeneration system and a Japanese cooling water recirculation pump.

Kounotori3?s Unpressurized Logistics Carrier is carrying more than 1,000 pounds of cargo to be attached to an experiment platform at the end of the Kibo module on August 6.

Kounotori3 launched from the Tanegashima Space Center in southern Japan on July 21. It will stay attached to the ISS until September 6 when, like its predecessors, it will be detached from the Harmony node by Canadarm2 and released for a fiery re-entry over the Pacific Ocean. The outer hull of the spacecraft is fitted with monitors to provide data about its re-entry.

HSBC says sorry as it takes $700 million hit

LONDON (AP) ? HSBC PLC apologized to shareholders Monday as it disclosed a $700 million charge to cover the cost of U.S. penalties for lapses including its failure to enforce money-laundering controls in Mexico.

The provision was announced as Europe's biggest bank reported an 11 percent advance in pretax profit in the first half of the year following $4.3 billion in gains from asset sales. For the six months ending June 30, the bank made a pretax profit of $12.7 billion, up from $11.5 billion a year earlier.

HSBC Mexico acknowledged in a statement that it failed to report 39 suspicious transactions and had been late in reporting 1,729 others.

The bank's chairman, Douglas Flint, and Chief Executive Stuart Gulliver apologized for the money-laundering infractions.

"We must demonstrate that we have learned from earlier mistakes," Flint said.

"The banking industry is operating in a hostile climate so we must double our efforts to convince our regulators, customers and investors that we are striving for the highest possible standards. Only that way can we allay public fears and regain trust in our industry," Flint said.

Gulliver said new processes and standards were being enforced in all of the bank's global operations.

"HSBC is now run and managed as a genuinely global firm, making it easier to set, monitor and enforce standards," he said.

As well as being dented by the money-laundering provisions, HSBC's first-half earnings were also hit by provisions of $1.3 billion for compensating U.K. customers for payment protection insurance.

However, the bank made no provision for settling potential regulatory fines and private lawsuits, including one pending in the United States, related to setting the London interbank offered rate (LIBOR) and other indexes.

In its program of getting out of non-strategic markets and non-core investments, HSBC has made 36 disposals and closures since early 2011.

A further look at the figures showed that operating income was up 3 percent to $43.7 billion while taxes paid more than doubled from $1.7 billion to $3.6 billion. However, net profit was down 8.6 percent to $8.4 billion, compared to $9.8 billion a year earlier.

HSBC shares were up 0.8 percent at 535.2 pence in late morning trading in London.

Although the first half profits fell short of analysts' forecasts, Jonathan Jackson at Killik & Co. said he rates the shares as a buy because HSBC has a "strong funding position, with excess cheap deposits, and is well capitalized."

Researchers from the University of Kentucky have designed and discovered a series of highly efficient enzymes that effectively metabolize cocaine. These high-activity cocaine-metabolizing enzymes could potentially prevent cocaine from producing physiological effects, and could aid in the treatment of drug dependency. The results of this study by Chang-Guo Zhan et al are published in the journal PLOS Computational Biology.

The effectiveness of the enzymes' work was evaluated through modeling cocaine pharmacokinetics, the study of the body's action on administered external substances, such as cocaine, when the enzyme exists in the body. As there is no FDA-approved anti-cocaine medication, the medical and social consequences of cocaine abuse have made the development of anti-cocaine medication a high priority. Administration of an enzyme to enhance cocaine metabolism has been recognized as a promising treatment strategy for overdose and abuse. A remarkable feature of the enzyme-based therapeutic approach is that one enzyme molecule can degrade many thousands of drug molecules per minute.

This pharmacokinetic modelling is a crucial step of enzyme-based therapy development for cocaine abuse. Furthermore, the general insights of the research should also be valuable for future development of an enzyme therapy for any addictive drug, as the general methodology of the modelling may be used to develop valuable models for evaluating the effectiveness of metabolic enzymes in detoxifying other drugs.

So in the spirit of all this impending violence perpetrated by the fairer sex, Cagewriter tracked down 125-pound female fighter Munah Holland, who knocked out Strikeforce veteran Carina Damm two weeks ago at Matrix Fights (a regional show in Philadelphia) to get her perspective on things.? She is, after all, on the front lines.

Things to know about Holland: she's a product of the Tiger Schulmann MMA system, and manages one of the organization's gyms in New Jersey.? In 2004, she won the prestigious Golden Gloves amateur boxing tournament, transitioned to professional fighting with Chuck Norris' ill-fated kickboxing promotion the World Combat League, and now fights in the cage, shimmying out of the clutches of Brazilian Jiu-Jitsu black belts and feeding them knuckles until they fall.? ?In her MMA debut, Holland brutalized Kim Couture for three rounds; more recently, she's KO'd top New England fighter Marianna Kheyfets in Bellator and put away Damm.

That's Holland.? Here's what she had to say about some of her fellow fighters:

On Strikeforce bantamweight champ Ronda Rousey - "Ronda is, I would say, this deliciously deadly, amazing person.? She has something that, when I watch her fight, is incredibly contained, and at the same time, explosive.? It's really fun to watch, and her ground game is sick.? She is definitely a force to be reckoned with, no matter her opponent in front of her.? I can't wait for the upcoming fight between her and Sarah Kaufman ? I think that's going to be a great fight."

On top Strikeforce bantamweight challenger and former champ Sarah Kaufman - "I've watched Sarah for a long time, because as a female martial artist, she was one of the girls I knew about first.? She's got really, really powerful hands ? like, lead in her hands.? She's very dangerous.? She's constantly evolving? got a lot of heart, and like Ronda, she's a force to be reckoned with.? And Sarah has some very good wrestling, so it's going to be interesting, because if Sarah can maintain her wrestling defense and use her hands and use her range, it could be a really interesting fight.? If it stays standing, Sarah can do some damage with her hands.? We haven't really seen Ronda's chin, we don't know how much damage it can take."? Her prediction for the August 18 Strikeforce title fight?? "If it stays standing, Sarah's going to have the advantage.? If it goes to the ground, my gut says Ronda."

On former Strikeforce bantamweight champ Miesha Tate - "I love Miesha.? I once said this out loud and people heard me, but if you have a bum that looks like Miesha's, you've got to plaster it everywhere.? She's beautiful, she's tough as nails.? And that fight with Ronda, that was sick."

On former Strikeforce featherweight champ Gina Carano - "Gina, I think, has done a world of good for female fighting in general.? She's chosen her own path, and gone the way she feels where her future lies.? You have to do whatever it is you have to do in this world to have a life and a good living, but I thank her for exposing female martial artists to the world, and making it important for people to watch and making it exciting for people to watch, and proving that, regardless of whether it's females or males, it's just a great sport to watch.? She proved that the numbers can be there, that the seats can be filled for female mixed martial artists just as they can for men."

On former Strikeforce featherweight champ Cris "Cyborg" Santos ? "Cyborg would scare most grown men away in the cage.? The first time I saw Cris Cyborg, I was like, 'Uh, this woman is just frightening.'? And frightening in a good way.? What a beast.? What a beast of mass proportions.? This girl? frightening strength, amazing ability coming from Chute Boxe, can really take so much.? She's just like a pitbull.? She's just going to keep on coming and keep on coming."

On female MMA pioneer Tara LaRosa - "Long before Gina or Cyborg came into the picture, you'd hear about Tara just making her way through one person after another in the circuit ? especially in New Jersey, where she's got a lot of fans.? For me, she was definitely an inspiration.? We can do it, we can do it really well and with a lot of class."

On hoping to fight in the UFC one day - "Oh yeah.? Some little girls want to grow up and wear pink slippers, but for me, the thought of being in the UFC?? I hope it gets to that point for women, I hope it gets to that point in my MMA lifetime.? We can only hope for that.? Who wouldn't want to be in that arena?"

On Dana White reconsidering his stance on not allowing women in the UFC's Octagon - "I don't know any woman who wouldn't want to see women in the UFC, and I hope Dana White considers that at some point.? Because we'll bring the crowds, we'll fill the seats, and people really enjoy watching it."

Toyota passes GM globally, and VW is closing fast

Toyota Motor Corp. regained the lead in global car sales over General Motors Co. in the first half of the year, with Volkswagen AG poised to move past GM and push the U.S. automaker into third place for the full year.

The bigger question is how soon the German auto giant also will pass Toyota and secure the global sales crown ? an internal goal that VW management has targeted for 2018 as part of the company's so-called Strategie 18.

VW, in a cautious statement released Thursday, said: "The Volkswagen Group is right on track for 2018, but has not crossed the finish line yet. For us, quality takes precedence over quantity. We don't want to be the biggest, but rather the best and most sustainable automaker in the world. For us, environmentally friendly products and satisfied customers and employees are at least as important as sales rankings and profitability."

It still has to maneuver past a resurgent Toyota, which has rebounded smartly from the effects of last year's earthquake and tsunami in Japan. Toyota in February said it expects to sell a record 9.58 million vehicles globally in 2012, shattering the old mark of 9.37 million in 2007.

Toyota had held the global sales crown from 2008 through 2010, before the natural disasters in Japan dropped it last year to third place, behind GM and VW.

"We have recovered greatly from the earthquake and tsunami," said Javier Moreno, a spokesman for New York-based Toyota North America. "Our dealers now have plenty of inventory and showroom traffic is up on a global basis."

Toyota said it sold 4.97 million vehicles worldwide in the first six months, a substantial increase from the 3.72 million it sold last year, when all Japanese automakers were reeling from the earthquake's aftermath.

GM said it sold 4.67 million vehicles in the first half, up 3.0 percent from 4.54 million the previous year. GM's global performance was driven by record first-half sales in China, which now accounts for more than 30 percent of the automaker's worldwide total. The U.S. market, where GM's sales are growing at a much slower pace, accounts for only 28 percent of the company's total sales.

GM spokesman James Cain on Wednesday said: "We continue to grow our sales and share in China . . . We are in the early days of the most aggressive roll out of new products in our history, which will help us press our advantage in the U.S. and China and grow profitably around the world."

VW, in a mid-year company report released Thursday, said its first-half sales totaled 4.64 million, up 12.4 percent from 4.13 million last year. But the German auto giant, which recently added sports-car maker Porsche to a diversified brand portfolio that also includes heavy-truck makers MAN and Scania, sounded a note of restraint.

"Deliveries by the Volkswagen Group developed very well in the first half of the year," sales boss Christian Klingler told reporters earlier this month. "But that is by no means cause for euphoria. The economic situation, particularly in Western Europe, remains tense and difficult."

Klingler added: "We remain on track and are entering the second half (of the) year, which will be altogether more challenging, with confidence."

A renewed focus on the U.S. market is part of VW's long-range plan to boost global sales to more than 10 million units by 2018, with the United States contributing at least 10 percent of that volume.

Tom Libby, senior automotive analyst with Southfield, Michigan-based R.L. Polk, said VW's management "has been very aggressive, with the U.S. playing a significant role in their global growth strategy. They have become more competitive on pricing here, and we expect their sales and share will continue to rise."

According to ?TechnoBuffalo?, which obtained the internal screenshot above, Sprint is set to launch the update to build no. L700.FG01 tomorrow, July 26.

According to the leaked image, the new software version will include changes to the on-screen signal strength indicator, as well as support for Sprint TV and some changes to the default Wifi and LTE settings. Both will be set to "on" as standard in the new firmware, which is to be expected as Sprint's recently-launched LTE network begins to grow.

A Coming Physician Shortage for Medicare and Medicaid Patients

While the ACA will cover more under government programs, many physicians refuse to accept such patients.

Editor's note: This is the third installment in "From the Trenches," a new monthly series that continues through October.

Now that the Supreme Court has affirmed most of the components of the Patient Protection and Affordable Care Act, many believe that the uncertainty is over and we can move on implementation. However, the experience with health care reform in Massachusetts shows that the uncertainty has just begun.

After six years of operating under a reform law similar to the ACA, Massachusetts finds itself wrestling with spiraling health care costs, including Medicaid costs that are now consuming 41 percent of the state budget. According to Jonathan Gruber, an MIT economist who helped craft both the state law and the federal law: "The state has no idea what's driving the growth in health care costs. In fact, it's not even close to figuring it out & . We don't know the answer, we don't know how to fix it now."

In response to these unanticipated, unsustainable health care economics, new emergency legislation has been introduced that would dramatically change health care delivery in the state. In its May 6, 2012, article, "Massachusetts Moves Toward Health-Care Price Controls. Is America Next?" Forbes profiled this new law. The 178-page bill, the Health Care Quality Improvement and Cost Reduction Act of 2012, introduces several new concepts to address the unintended crisis resulting from the original health care reform law, including:

  • a new, quasi-governmental oversight agency called the Division of Health Care Cost and Quality;
  • a cap for health care spending linked to the local economy, the Gross State Product, minus one-half a percent;
  • a 10 percent luxury tax on hospitals that charge more than 20 percent of the state median price for a given service unless they can justify that higher price.

Many of the issues that are being addressed by this emergency legislation were predictable when the original law was passed. But rather than anticipate these problems and address them in the original legislation, Massachusetts chose to believe the most optimistic projections and ignore the realities of the market until they have become a crisis.

While the ACA addresses coverage for all (unless a state opts out of the new Medicaid coverage plan), it is likely that the law will accelerate a crisis regarding physician access for Medicaid and Medicare patients. Evidence of this pending crisis is already percolating in the trenches.

Medicare/Medicaid Patients: Good for Business?

Mike Leavitt, former Health & Human Services secretary and leader of Mitt Romney's health care transition team, recently stated that "we have entered into an era of dispassionate health care, where it's about business and survival & ." This seems to be the consensus of most physicians as well. In a recent survey of physicians reported in CNN Money online (June 27, 2012), doctors reported that it is getting much harder for them to earn a decent living, especially in private practice. The reasons given for this increase in financial hardship include: education debt, rising business expenses and administrative hassles, shrinking reimbursement, and costly malpractice insurance.

This cry for help from physicians was articulated in a March 28, 2012, letter from more than 50 physician organizations to Marilyn Tavenner, acting administrator of the Centers for Medicare & Medicaid Services, which stated: "The undersigned organizations are writing to express our profound concern about the imminent storm that is about to occur due to simultaneous implementation of multiple programs that will create extraordinary financial and administrative burden as well as mass confusion for physicians."

The growing disparity between Medicare and Medicaid and commercial rates is a threat to physician income. CMS's Office of the Actuary reports that in 2009, Medicare payments were about 80 percent of private health insurance, while Medicaid payments were 58 percent. However, the gap between government reimbursement and commercial rates is significantly greater for dominant medical groups who are able to negotiate better rates. Given the wide disparity between government and commercial reimbursement, it is not surprising that Jackson Healthcare's Physician Practice Trends Survey 2012 reports that:

  • 36 percent of physician respondents reported being unable to accept new Medicaid patients;
  • 26 percent of physicians said they do not see Medicaid patients at all;
  • 17 percent of all physician respondents reported being unable to accept new Medicare patients;
  • 10 percent of physicians said they do not see Medicare patients at all.

This is not news to those of us who negotiate physician transactions for a living. More often than not, physicians begin their negotiations by explaining that they are in "the business of practicing medicine." Their implication is that the success of the negotiation and their willingness to engage with the health care system (and open their practice to Medicare and Medicaid patients, take call, etc.) is contingent on first satisfying their concerns about their desired level of compensation in the face of low government reimbursement and new regulatory initiatives.

Numbers That Don't Add Up

A recent discussion with the physician leader of a large, prestigious surgical group in the Southeast about why his group decided to stop seeing new Medicare and Medicaid patients is representative of physician sentiment in the trenches:

First, he pointed out that there is a severe shortage in his specialty, and his group cannot keep up with the demand from the commercially insured population. He explained that the practice overhead for treating Medicaid patients is greater than the reimbursement his group receives from the state (whenever the state has the funds to pay them). With Medicare patients, their conditions are more complex and time-consuming than those of the commercially insured population, and Medicare pays his group about 60 percent less than what they get paid for the equivalent service under commercial insurance.

Had payment rates been his group's only issue, they probably would have kept their practice open to a limited number of Medicare and Medicaid patients. But the complexity and consequences of the new regulations (e.g., the recovery audit contractor and physician quality reporting system programs, etc.) were the tipping point that moved the group to close their practice to new Medicare and Medicaid patients.

The gap between commercial and Medicare reimbursement rates will get even worse if CMS' proposed 2013 fee schedule goes into effect. Most commercial insurance companies are providing physicians with a modest increase of at least 2 to 3 percent. But in exchange for marginal increases in primary care rates (which will have minimal impact on the physicians' after-tax income), CMS is proposing 2 to 4 percent reductions in the fee schedules for radiology, anesthesiology, cardiology, pathology, vascular surgery and urology. In addition, CMS is proposing no increase for allergy/immunology, gastroenterology, general surgery, plastic surgery and rheumatology.

One common theme in every health care system is the demand by local physicians to provide funds to stabilize their incomes at market rates. Given current regulations, it is difficult for health systems to protect the incomes of physicians in private practice from declining reimbursement or to provide direct incentives to independent physicians to see Medicare and Medicaid patients.

Thus, the health system must use popular physician engagement strategies such as joint ventures, call pay, directorships or co-management agreements. In many cases, these engagement strategies will not provide sufficient cash flow to stabilize a physician's current level of income over the long term. It is also important to note that these engagement strategies usually represent an incremental cost to the health system, e.g., payments for an emergency department call.

Health systems that compensate their employed physicians based on collections minus expense are finding that these physicians are limiting the number of Medicare and Medicaid patients in their practice unless they receive a subsidy for seeing these patients. Most often, this risk of low government rates is mitigated for employed physicians by using a compensation system based on a market rate per wRVU. Under this popular compensation formula, the health system shelters the physician from the unfavorable economics on government-funded programs and absorbs the losses associated with this subsidy.

Unbridled Growth

The primary economic factors that have mitigated physician access problems for Medicare and Medicaid patients are the engagement and employment subsidies provided by health systems. However, the health systems' ability to afford these subsidies over the long term is in doubt. In a recent interview, Medicare's chief actuary, Richard Foster, said that while he's optimistic that providers can improve the quality of care and lower costs, he expressed doubt that the growth rates can be reduced over the long term. In his congressional testimony Foster stated:

"Many ideas have been developed and tried over the years in an effort to reduce health care cost growth. Examples include the development of prospective payment systems and other bundled payment mechanisms; the widespread adoption of managed care plans; efforts to facilitate more prudent use of health care services through consumer-driven health plans and medical savings accounts; use of Lean production techniques by hospitals and other facilities; and, most recently, the development of accountable care organizations, medical homes, disease management and other efforts to better integrate the delivery of care. Most of these efforts have had some positive impact on lowering the level of health care costs, but there is relatively little evidence that they have succeeded in reducing cost growth rates."

Foster also notes that without unprecedented changes in how health care is delivered and payments are made, "the prices paid by Medicare for health services [per the ACA productivity adjustment for hospital inpatient services] are very likely to fall increasingly short of the cost of providing these services." Over time, he projects that payments for some services would be halved, forcing Medicare payments [to hospitals] below Medicaid rates.

As Moody's notes in its June 28 special comment on nonprofit hospitals, the key longer-term, credit-negative effects of the ACA on nonprofit hospitals are as follows:

  • Spread over 10 years, Medicare reimbursement payments to hospitals will be reduced by more than $150 billion in addition to $14 billion of Medicaid disproportionate share funding cuts.
  • There will be new forms of reimbursement models (such as bundled payments) that also seek to lower reimbursement to hospitals.
  • New payment penalties for hospitals with high readmission rates particularly will affect inefficient and poor-quality hospitals.

In addition, regulation of commercial premium increases will have a dramatic effect on a health care system's ability to raise its commercial rates. Clearly, these cuts will impact the ability of health systems to subsidize physicians seeing underfunded government patients.

The Coming Physician Shortage

The Association of American Medical Colleges predicts that by 2025, the supply of physicians will fall 130,600 short of demand. Given the ever-increasing disparity between government and commercial rates, the law of supply and demand suggests that without a subsidy from the health system, physicians will further limit access to Medicare and Medicaid patients. But the ACA will place more pressure on hospital rates, thus threatening the ability of health systems to subsidize physicians for seeing Medicare and Medicaid patients.

Lack of access to physicians for Medicare and Medicaid patients will be one of the first and most predictable unintended consequences emerging from implementation of the ACA. This is a complex problem that requires the immediate attention of the health care policy community today; unfortunately, it is not getting the attention it deserves.
